Registration for Faith Formation Classes for

Children and Youth for the

2018-2019

 

First Day of Registration is Sunday, June 10, 2018

To register your child, the following items MUST be submitted to the Faith Formation Office:

​

  • Fully Completed Registration Form: Parents must read and acknowledge each of the Policies on the registration form. Only the mother, the father or the legal guardian can sign the registration form.

  • Baptismal Certificate: to register your child (Birth Certificate if the child is not baptized). For the Confirmation Program you also MUST bring 1st Communion Certificate.

  • Family Catechesis Commitment Form: (ONLY for 1st Communion Program)

  • Full payment: Make Check or Money Order payable to “St. Camillus Faith Formation” or make cash payment.

REGISTRATION FEE: Discount ($15) during Sunday, June 11 - Friday, July 28, 2017

  • Intermediate Levels (Godly Play, Kinder and grades 3rd – 6th):

    • $75 for 1st child. $15 for each additional child in an Intermediate Level.

  • Sacramental Programs (First Communion, Confirmation and RCIY):

    • $120 for 1st child. $40 for each additional child in a Sacramental Program or $15 for each additional child in an Intermediate Level. (Maximum of $200.00 per family).

Payment Plans – If a financial hardship exists, please contact the Faith Formation Office to make an appointment with the Director to make a payment plan. No child will be turned away from the Faith Formation Program due to a financial hardship. All tuition arrangements will be held in strict confidence.
